In today’s healthcare landscape, seamless communication between care teams is crucial for delivering the best outcomes. This project focused on creating an innovative platform that bridges gaps in care, streamlines workflows, and empowers providers with the tools they need to collaborate effectively.

The goal was to design a solution that supports physicians and hospital systems in transitioning to value-based care. The Patient Tracker application specifically focuses on monitoring patients after hospital discharge, ensuring they smoothly transition to their next acute-care facility and reducing the risk of readmissions.

We began with an in-depth discovery process, engaging healthcare professionals to understand their workflows, challenges, and priorities. These insights shaped the design strategy, which focused on intuitive navigation, customizable dashboards, and data integration that provided a 360-degree view of patient care.

The platform’s interface was crafted to feel familiar and user-friendly, reducing the learning curve while enhancing productivity. Interactive features, such as task management and care alerts, helped teams stay aligned and proactive. The design system ensured visual consistency and scalability, enabling seamless adaptation to future expansions.

Collaborating closely with product and architecture, we ensured the solution not only met technical requirements but also fostered meaningful connections between care providers.
